	Please fill out the registration form and order form (above) and send it in with your prescription. Make sure your doctor’s license number and phone number is written on the prescription. If you do not have your original prescription have your doctor or pharmacy fax it to us. If you have no access to a fax, mail to: #770, 2710 17 Ave SE, Calgary, AB Canada T2A 0P6 (Note: Sending the Order Form by mail may delay your order by 7-10 days).
